Our third-party partner has identified an issue and is preparing to release an update to the services. During this update, the form will be unavailable. We’ve been assured that this update should be a very brief window of time, and after the release, we can expect more normal loading experiences.

We’ve seen improvement in load times and experiences for individuals. Our third-party partner continues their work to resolve the issue.

Our Engineering team has been working with our third-party partner to identify the cause of the latency issues. While we’re seeing some improvement to loading times, individuals may see atypical loading behavior. We’ll provide another update as soon as possible.

Individuals are reporting delayed loading or loading of a blank page when attempting to access the COVID-19 Health Check. Our Engineering team is treating this as their highest priority.
